Before you visit with our doctor, your animal must be trained to provide you a medical service in order to qualify for the letter. We cannot perform the exam if your animal is not trained or if the animal is in the process of being trained.
Service Animals are protected by the Americans with Disabilities Act, Fair Housing Act, and Air Carrier Access Act.
Effective January 11, 2021: Only service dogs are allowed for air travel. If you wish to fly with an animal that is not a dog, pet fees may apply.
What is the process for a service animal evaluation?
1. Fill out a medical history form, consent form, and an animal behavior agreement for our doctor to review.
2. Visit with our doctor. We offer both in-person and telehealth evaluations. During your visit, our doctor will perform an evaluation to determine if you qualify for the service animal letter.
3. If the doctor determines you qualify for a service animal letter, you will receive that letter following payment. This letter will be valid immediately and can be used right away.
